Three, about a week apart in our practice, with a review at six months. Each session is eight minutes of treatment inside an appointment of about 30 minutes in our practice, which covers the examination, the consent and the aftercare. The manufacturer publishes no course of its own. The one published EMfemme 360 study used three sessions a month apart. BTL's own registered trial, which paired it with the Emsella chair, spaced those combined sessions five to ten days apart, so a week sits inside what has been used. At EMfemme 360 London, 8 Devonshire Place, the doctor examines you first and the course is in writing before the first session.
Why three
Radiofrequency warms the tissue to 40 to 45°C, the range in the US clearance summary, and BTL's account is that the controlled heat stimulates collagen production, and the study's authors suggest it also improves blood supply. In our clinical judgment that response builds with repetition, which is why the treatment is given as a short course rather than once. In the published study, the three sessions took pain on intercourse from present in all 35 women to absent in 29, and the vaginal health score from 7.7 to 18.7 on a 5 to 25 scale, measured two months after the third.
What a session involves
The doctor examines you at the first visit and checks the list of things that rule treatment out. For the treatment itself a single-use tip is fitted to the handpiece and moved slowly inside the vaginal canal. An integrated thermometer shows the tissue temperature and the device adjusts the power; most women describe warmth, and the practitioner turns the energy down if you ask. Eight minutes. Where the outside is treated as well, over the labia, the vaginal opening and the perineum (the skin between the vagina and the back passage), the manufacturer's clinical guide describes three to five minutes a zone. You dress and leave; in our practice we advise no intercourse, tampons or baths for 24 to 48 hours. A female practitioner can deliver the treatment if you ask.
When a session is delayed
A session is not given during a period in our practice, with thrush or a urinary infection, with unexplained bleeding, or when you are trying to conceive; the doctor says so and the course is moved. The FAQ lists the main things that rule the treatment out, and the doctor goes through the full list with you.
After the course
The review is at six months with the doctor, who scores the same things as at the start. The published study followed women for two months only, and the manufacturer gives no maintenance schedule; the results page sets out what is and is not known.
